DOT physical
We offer routine physicals required by the DOT (Department of Transportation) for CDL motor vehicle drivers.
If you have come at the request of your employer, you should bring a completed and signed Authorization Form that lists the desired services (this will be given to you by your employer), as well as proof of identification.
If you are choosing to come to Carbon Health for workplace health services, please bring proof of identification and a method of payment.
