FAQs

  • How do my employees schedule their appointments?
    Employees can scan the QR code on the Carbon Health authorization form or book directly via our website: carbonhealth.com/get-care/workplace-health. Booking instructions are also provided in our Welcome email to be shared with your employees.
  • What do my employees need to bring to their appointment?
    They will need to provide a signed Carbon Health authorization form that lists the requested services (you will be provided with a copy of this authorization form once your account is set up to share with your employees), as well as proof of identification.
  • Where can I find an authorization form?
    A copy of our Carbon Health authorization form is provided to your organization in the Welcome Email once your account is created. An employer must complete and provide this form to their employees prior to their visit.
  • How and when will I receive results?
    Your company will receive access to your unique Employer Portal and a designated representative will receive email notifications as results are uploaded to the Portal.

    For non-lab results and in-house lab results, same-day results can be expected. For send-out labs, the turnaround time is 3 to 5 business days.
  • Who pays for the visit?
    During the first week of each month, employers will receive an email notification to view and pay their invoice in our Employer Portal.
  • Is my employee’s health information secure?
