- Severe pain during menstruation can be caused by several factors, including medical conditions and hormonal imbalances. The most common cause of severe menstrual pain is a condition called primary dysmenorrhea, which is caused by an increase in prostaglandins, hormone-like substances that are released during menstruation, that cause the uterus to contract and may cause pain and discomfort.
