Carbon Health board certified providers are highly skilled in Occupational Medicine and in workers’ comp and post accident evaluations. We provide care for injuries that occur while you're on the job to get you back to work safely. Mindful of OSHA recordables.
If you have come at the request of your employer, you should bring a completed and signed Authorization Form(opens in new window) that lists the desired services (this will be given to you by your employer), as well as proof of identification.
If you are choosing to come to Carbon Health for workplace health services, please bring proof of identification and a method of payment.
